Ebstein’s anomaly
A congenital heart defect characterised by displacement of septal and posterior leaflets of the tricuspid valve towards the apex of the heart. Anterior leaflet is variably affected.
The displacement itself results in tricuspid regurgitation. Additionally, the atrialised portion of the right ventricle continues to contract with the right ventricle, contributing to regurgitation and stasis of blood within the right atrium.
Seen in SLE. Vegetations consist of inflammatory cells and fibrin, and can occur on either side of valve flap surface.
Pericarditis secondary to injury to heart (endocardium, myocardium, pericardium), e.g. MI.
Congenital L-to-R shunt progressing to R-to-L shunt due to pulmonary HTN and right ventricular hypertrophy (R heart pressure > L heart pressure).
